Provides emergency housing through the Supervised Family Residence (SFR). The SFR houses up to nine families each, living under the supervision of a case manager. Families are able to give each other peer support and encouragement, and everyone benefits from the homelike atmosphere. CHI also provides emergency housing to single adults in a Supervised Singles Residence (SSR), licensed by New York State, using the congregate model and providing intensive case management, including supervision from a psychiatric nurse practitioner. In addition, CHI in partnership with Suffolk County DSS, operates a service center in Patchogue open to homeless single adults. Clients at the Resource Center receive help with applying for benefits, addressing mental and physical health issues, education and finding employment.

The goal of the program is to help this special group of students get accepted into the school of their choice in order to pursue a career in medicine. Throughout the program, students receive personalized academic and application advisement through the Office of Pre-Professional Advisement and Fellowships. Prior to being enrolled students may transfer in a maximum of 12 credits in to the certificate program. Candidates for this program must have completed at least 24 credits.
