Migrant Health Services is a collaborative service offered by the County Health Department and the Hudson Valley Migrant Health Program of the Hudson River Community Health Center that provides outreach and education to migrant farm workers in Orange County to reduce their risk of contracting or spreading communicable diseases and to improve occupational health and safety.

Housing Services for Senior Citizens and Disabled Adults is a 105-unit assisted living program with a secure dementia unit. They accept seniors and younger adults with disabilities. Assisted living programs provide residents of adult homes or enriched housing, with home health care, in addition to the services regularly available within an adult home or enriched housing. Assisted living programs are regulated and licensed by the New York State Health Department.

It aims to improve access to necessary healthcare and stabilize living conditions for eligible residents of Suffolk County, helping them manage their health and well-being effectively.

Is a NYSDOH grant funded project designed to improve the health of women ages 12-44 and their families. The Suffolk County MICHC project has chosen to focus on 10 high risk zip codes located in Western Suffolk County: Amityville, 11701; Babylon, 11702; Bay Shore, 11706; Brentwood, 11717; Central Islip, 11722; Commack, 11725; Copiague, 11726; Deer Park, 11729; Lindenhurst, 11757; North Babylon, 11703; West Babylon, 11704; and Wyandanch, 11798. In these areas, the effects of racial, ethnic and economic disparities are evident. The Suffolk County MICHC embraces strategies and activities that address maternal and infant health behaviors at multiple levels, by addressing women’s health across their life course. We aim to help women be as healthy as possible before, during and after pregnancy. MICHC will provide education, case coordination and home-visiting services to pregnant and parenting women, and coordinate health and social service linkage and follow-up for low-income women, teens and families to increase utilization and continuity of care. They also provide group and individual education. The MICHC initiative will develop and facilitate a collaborative of local health, social service and community providers to create a referral network to increase service access and to identify gaps in service. The Collaborative also works to identify, address and improve barriers to health for women of child-bearing age. Suffolk County MICHC will work with our partners and community leaders to improve community conditions and opportunities that make it easier for people to make healthy choices. The Fulton County Office for Aging provides information about the many community services and benefits available to senior citizens.

Accepts household items including kitchenware, small kitchen appliances, linens, winter coats and boots, baby clothing, some furniture, children's books and books for English learners and dictionaries as well as gift cards to grocery and department stores.Unable to receive: televisions, furniture, washing machines or dryers, stoves, decorative items, clothing and books not listed.
