Registrants can also request to receive an identification bracelet by checking the box on the Special Needs/Silver Alert Registry form. The bracelet is free of charge and contains a distinctive I.D. number as well as a Suffolk County Police Department phone number which first responders can utilize to obtain information regarding that person. The Children and Youth with Special Health Care Needs (CYSHCN) Program offers supports to CYSHCN and their families by providing timely and appropriate information and referrals to insurance, health services, and community resources to address their identified needs. The program is available to families with children and youth, ages 0 to 21, who have a chronic physical, developmental, behavioral or emotional condition and may require health & related services.

Experience case workers and nurses help functionally impaired Schenectady county residents with in-home services in the least restrictive setting with the assistance of family, friends and communities. The services are an access point for agencies and the community to obtain information about community-based services including long-term in-home care. The services Schenectady County Long Term Care unit may authorize are: Personal Care Service Program, Consumer Directed Personal Assistance Program, Private Duty Nursing, and Long Term Home Health Care Program.
