Palliative Care is a recognized medical specialty which is designed to create a collaborative, holistic plan that includes the patient, the family and/or caregivers, and the involved providers, addresses the medical, psychosocial and and spiritual needs of a patient and family, provides an opportunity to discuss, review and document wishes in regards to future healthcare and life sustaining treatment decisions. It is designed to be offered to conjunction with curative treatment, ideally at the diagnosis of a chronic or life-limiting illness as a supportive means to ensure quality of life for both the patient and the family. When appropriate a transition to a plan for Hospice or Comfort Care can and will be supported.

Food pantry available to community members in need of food. Items include (but are not limited to) non-perishables, dairy items, fresh produce, and variety of meats. It is set up like a grocery store where people are given a shopping cart and allowed to take what they need.

Offers school supplies for students, preschool through 12th grade. Note: This drive is given during the month of August.

Congregate meals serving seniors in Clinton County.

Provide daytime or evening access to heated facilities during times of extreme cold for people who are temporarily at risk for exposure due to a power failure, fuel shortage, road closure, homelessness or other situations which make them vulnerable. Code Blue warming site is activated when the windchill and/or temperature reaches 15*F or below. Law Enforcement agencies or any other organizations that come in contact with or becomes aware of any individuals that are in need of sheltering can bring them to the warming center. Activation date and time varies.

Offers a diabetes clinic which helps individuals to manage their disease and educate them about the various aspects of keeping it under control.

Offers a Stroke Survivor Support Group which meets virtually via Microsoft Teams. Call for detailed information.

Provides small group homes, certified as Individual Residential Alternatives to persons with developmental disabilities, multiple physical handicaps, ambulatory and non-ambulatory, ages teen through adult. Homes located in Nassau, Suffolk, Queens Counties and New York City.

Provides a point of access that directs adults to mental health or substance abuse services in the community.

The building department monitors new building conditions and construction, and oversees the maintenance of older multi-family structures. Also enforces the state and local building codes, housing standards and ordinances, to the review of applications for permits of new buildings, repairs, alterations, signs, and rooming houses, in addition to electrical and plumbing work. The building department accepts complaints from tenants about conditions the landlord fails to resolve.

Provides grocery bags of food for households each week. Food items come from Community Food Banks (Island Harvest, LI Cares Food Bank, and other donors).

The Community Foundation provides grants to not-for-profit organizations in Dutchess, Putnam, and Ulster counties manages endowments set up for charitable purposes, including scholarship funds, and offers the Partnership in Education Grants Program grants to teachers for professional development and/or special projects to be accomplished with their classes.

DMV provides first time drivers licenses and permits, non-driver's licenses (identification cards), renewing driver licenses, vehicle registration, road tests, replace lost titles or licenses, and other driving-related issues. Many tasks can also be done online.

Offers a clinic and support care for people diagnosed with diabetes, to help cope and manage their disease.

Offers a twelve-Step program for anyone who has been affected by a friend or family member's drug addiction. Meeting participants share their experience, strength, and hope, and letting new people know they are not alone. Information is made available through public information, hospitals, institutions and websites.

Operates Pre-K through 6th grade classes.

Handles legal matters including civil, criminal, small claims, and eviction cases within the Town of Mohawk.

Provides a free public education to youth in grades 6th through 8th.

Clinical services include: Obstetrics routine prenatal care and delivery; High-risk pregnancy care; Genetic counseling & nutrition counseling available; Prenatal classes and maternity tours; Postpartum education counseling; Dental consultations; Social work and Free pregnancy tests. Offers a broad range of peer-run self-help groups dealing with various issues.

Provides a service, in collaboration with 2 other agencies (My Sisters' Place and Westchester Hispanic Coalition) to offer free legal services for poverty level residents or those who are victims of domestic violence for consumer disputes, housing, education, benefits, domestic violence, orders of protection, custody, child support, family law, including citizenship applications, work authorizations or other immigration issues.

Provides a 7-day a week adult day program which includes nursing care, physical, occupational and speech therapy, social work, recreational activities, outings, meals, nutrition education, and wheelchair accessible transportation.
