Offers an annual, set amount of funds to people or families of people with disabilities to spend on services or goods not covered by Medicaid. The purchased goods or services must either enhance the skills of individual, improve their home environment, or help with their cognitive or physical development. Funds may be used to hire a respite worker. If the individual applying is also seeking a residential placement they could be eligible for an enhanced reimbursement amount. This money is intended to assist the family to keep their family member in the home and meet their needs while awaiting placement.

  • Will listen to residents' concerns, comments, and opinions.
  • Will advocate to ensure the highest quality of care for residents of long term care facilities.
  • Will work to resolve conflicts between residents and the long term care facility and help to develop collaborative solutions and resolve resident related concerns with the long term care facility.
  • Will investigate complaints and make appropriate referrals.
  • Will analyze, comment on and monitor the development and implementation of Federal, State and local laws pertaining to the residents of long term care facilities.
  • Will educate residents, family members and staff about resident's rights and available resources.

Positive behavioral support. Life safety, hands-on-assistance provided by staff as necessary. Professional services as necessary. Self-advocacy. Informed choice, community inclusion, and relationship building.

Provides home health services including assistance with bathing, dressing, and grooming, nursing assessments, homemaker services, wound care assistance, housekeeping, and grocery shopping.

• Provides a range of supportive services for individuals diagnosed with breast cancer.

• Provides civil legal services to low-income individuals. Family law. Domestic violence issues. Tort defense. Bankruptcy. Immigration. Unemployment insurance denial. Consumer problems. Real estate. Tax controversies. * Assists with guardianship/custody of children, wills and health care proxies for parents who are HIV-positive.

• Provides confidential diagnosis and treatment of STD's.* Offers HIV counseling and testing. * Provides pregnancy testing.* Provides applicable vaccines.* Provides medications to keep HIV negative people from becoming infected (PrEP).

• Offers independent living housing for older adults or those that are mobility impaired.

• Offers clinical services including: Evaluation. Treatment planning. Individual counseling. Certified one-on-one peer coaching. Group sessions. Family and youth counseling. Relapse prevention. Recovery planning. Other co-dependency programs. Aftercare services. Care management. Home and Community Based services. COTI/PEER.

• Teaches blind or visually impaired individuals skills of daily living such as cooking, sewing, self-care and communication skills, including the use of talking book machines, Braille, script writing, typing, etc.* Provides work site evaluations.
