The Emergency Assistance program guides a client through 'the system' and when other resources are not available and the client meets specific guidelines as outlined by Dutchess Outreach, they may be eligible for one time financial assistance once per lifetime including utility payment, prescription drug assistance, etc. For prescription assistance, client must provide original prescription no refills. Only provides advocacy for NYSEG customers. Central Hudson customers must meet all of the eligibility criteria in order to receive financial assistance. Clients encouraged to call to receive information related to if funds are exhausted. Funds are distributed on case by case situation depending on funding and other eligibility criteria. The North Country Chronic Disease Coalition’s goal is for each Health System Service Area to have developed and started implementation of a Community-Clinical action plan. These plans will create and/or increase awareness of health-promoting resources that providers can refer patients to for community-based chronic disease risk reduction activities and support.

Act as a liaison between the school administration, students, and the Police Department. SROs provide a safe learning environment in schools, contribute valuable resources to school staff members, and foster positive relationships with youth and the community. SROs work with school officials in identifying at-risk youth, then mentor and guide them to resources within their community to help form more positive behaviors and choices. SCPD has a Spanish speaking SRO assigned to the Community Relations Bureau to help foster relationships with the Spanish speaking community within Suffolk County. School Resource Officers conduct presentations to high schools and middle schools across the county regarding various safety topics, including, but not limited to, Internet Safety, The Dangers of Drugs and Alcohol, Vehicle Safety, What To Do When Stopped by the Police and Opportunities of Employment within the Suffolk County Police Department. SROs also conduct programs in Elementary Schools such as 911 Awareness and Stranger Danger. SROs participate in after school activities such as sports, cooking, hiking and fishing. SROs also organize field trips such as Police Week and assist at college visits with school personnel, providing students with the ability to develop a close relationship with their SRO. Programs can be tailored and every effort will be made to accommodate the needs of each individual school based on current events and local issues. Provides a variety of summer camp opportunities for school-aged children.

Provides volunteer opportunities. Volunteers are compassionate, caring individuals who help provide a positive hospital experience for patients and their families. They serve in a number of capacities, including assisting in clinical departments or at the hospital gift shop. To volunteer, you need to be 15 years of age or older and able to commit at least 3-4 hours per week for a minimum of six months. Positions are available seven days a week with a variety of shifts. We try to match volunteers with opportunities based on their interests, talents and availability. But assignments change often and not every position is always open. We welcome high school and college students during their summer break. It is a great opportunity to gain insight into a medical setting. Summer program volunteering spans from mid-May/early June until mid-to end of August. Spots are limited and fill quickly. We recommend applying no later than mid-spring in order to be considered. All positions require students to be professional and mature, able to work with other volunteers and staff, and provide great customer service. Call for detailed information.

Whitney M. Young Health Services provides podiatry services to the uninsured and working poor residents of Albany and Rensselaer Counties. Podiatrists improve the overall health and well-being of patients by focusing on preventing, diagnosing, and treating conditions associated with the foot and ankle.

Offers case management support to parents to assist with areas such as skill building, crisis management, parent education, budgeting, emotion regulation, time management, communication, and community linkages. Clients are seen at least once a week in their home with services that utilize a trauma informed approach.

Provides home visits to identify environmental hazards and educate families and provide referrals. Distributes free cleaning, safety, and health products to improve the home environment

Provides professional support, education, information and resources to bereaved in the community via support groups for adults, children and teens. Memorial services, referrals, community education counseling and community crisis intervention are also provided.

Provides various residential housing in neighborhoods throughout the county, such as supportive apartments, supervised apartments and family-style group homes, which include clinical, medical, recreational and educational services.

Provides financial management and training to assist individuals who receive SSI/SSD benefits to live independently.

Offers a model of the Parent Child Home Program which is essentially the same as the home-based Parent - Child Home Program except that the recipients of the program are homeless families. The program is designed to stimulate the development of educationally at-risk Pre-K children. Trained home visitors demonstrate to parents/caregivers how to talk and read to their two and three year old children while staying in temporary housing. They give developmentally appropriate books and toys to each family. Home visitors provide consistent service to children and families for 2 – 3 years. This family literacy program is delivered to homeless children ages 18 months to 4 years old and their parents. It provides on-going educational services for each family with the same home-visitor for 2 – 3 years. Children who complete the Parent Child Home Program enter school ready to learn and graduate from high school at the same rate as middle-income students. Call for more detailed information.

Promotes the healthy practices of washing hands, and other actions, and how effective these simple things can stop the spread of viruses and some diseases.

Provides Information and Referral, Reference Services, Readers Advisory and programs for children from birth through grade 8 as well as parents. Programs and services include, but are not limited to, topics such as parenting workshops, storytimes, reading clubs and arts and crafts.
