Provides substance abuse evaluation and treatment services including individual, group, and psychiatric care. Also offers treatment for co-occurring disorders. Does not provide detoxification services. ##REENTRY##

Recruits, trains, and supports foster parents including those providing therapeutic foster care in kinship homes, traditional two parent homes, single parent homes, same-sex couple homes, and racially diverse homes. Services include the following:

• Offers emergency support 24 hours a day, 7 days a week.
• Offers monitored visitation with birth family.
• Offers Licensed Social Worker and Mental Health professionals as well as case managers.
• Offers support groups and training to Foster Parents as well as respite (babysitting) if needed and assistance in all areas of care.
• Provides psychiatric services when needed.
• Practices and trains in trauma informed and trauma sensitive environments and approaches.

A network of over 85 housing counseling and legal services organizations across the state that provide free, qualified mortgage assistance relief services to homeowners. HOPP representatives can connect New Yorkers to high-quality counseling or legal services all over New York State. The HOPP Program is designed to ensure that no family in New York State ever loses their home because they do not have access to a lawyer or qualified housing counselor. Services include:

• Assistance with applying for a loan modification, or other types of loss mitigation
• Direct representation in court proceedings, including settlement conferences
• Homeownership counseling

Operates HomeownerHelpNY (https://homeownerhelpny.org/), an online service that provides New Yorkers with the tools and support to avoid foreclosure and/or scams, and to help them find qualified mortgage assistance relief services from the HOPP network. HomeownerHelpNY is a collaboration between the New York State Attorney General Office and the Center for NYC Neighborhoods, a non-profit committed to protecting and preserving affordable homeownership.

Provides Disability Awareness Training for first responders to help them serve individuals with disabilities.

The New Dawn Initiative, sustained through Peaceprints of WNY’s Project Blue program, provides reentry-focused support for high-risk incarcerated individuals with co-occurring mental health and substance use needs. Participants work weekly with a case manager to assist in the development of individualized case plans and can engage in services offered directly through Peaceprints and community partners or through linkages to community-based organizations. Available supports may include housing assistance, education and employment services, mental health and substance use treatment, public benefits assistance, and other reentry-related services. Case management services continue for up to one year following release. ##REENTRY##

Certifies adoptive homes that will provide a stable, safe, loving and positive home environment for children. Program offers: recruitment, orientations, training, home study evaluation, pre/post placement services, advocacy and parent/peer support groups.

A 25-bed residential treatment facility located on the Horizon Village campus that provides specialized behavioral treatment services for 18-25 year old males with substance abuse and mental health issues. ##REENTRY##

Provides secure, anonymous disposal kiosks at sites throughout Erie County to safely and conveniently dispose of used needles, syringes, and lancets, free of charge. Disposal kiosks are found in many pharmacies, police agencies, Community Based Organizations, and Diabetes Specialty Care locations.

Works to enable low income persons to meet health costs. Medicaid is the state and federally funded health insurance program designed to assist low income individuals and families obtain medical care for themselves and their children.

Permanent, affordable, independent housing service for adults with emotional/mental illness. Housing service includes:

• Assistance in locating quality housing
• Subsidized rent
• Assurance that housing is furnished and appropriately equipped
• Linkage and coordination with community based services
• Tenant advocacy / landlord relations assistance
• Planned transportation

Serves as a site for various providers offering classes for those wishing to further their education or get better jobs. Offers classes preparing students for jobs in fields such as Healthcare, GED, ESL, Information Technology, and Construction Trades. Also offers test prep and tutoring in basic math and basic English.

Family centered process that employs the wraparound facilitation model to help preserve the family unit through the coordination of services including respite, skill building, crisis intervention, family intervention, and transportation.

Provides assessment and supportive counseling to assist individuals to maintain mental and emotional stability, remain safely in the community, and avoid hospitalization.
