Health education is provided by the Community Health Nursing staff, the Community Health Worker, and the Children with Special Needs staff. Education takes place on home visits, during clinics, during various health fairs and community events, in coordination with other departments, and through the media. Offers a program which is a free resource for seniors to get information or guidance when faced with health and care issues. The program provides assistance to older adults, families and caregivers who need help answering healthcare questions, identifying their needs, and accessing necessary services. Some of these services are: Social Interaction/Programming; Cognitive Programming; Nutrition; Benefits Assistance; Home Services and Housing/Placement. Stony Brook Eastern Long Island Hospital is working closely with Southold Town Senior Services and other local programs that serve seniors; and is committed to strengthening our community by improving the quality of life for older adults. Offers one-to-one mentoring for school-aged youth at-risk of failing in school, dropping out of school, or becoming involved in delinquent behavior. The goal is that of reinforcing the youth's self-esteem, self-worth and reinforce positive self-image, bolster positive motivation and character development through a positive role model example. Our mentors serve as a friend, coach and guide to mentees with a commitment to offer support, encouragement and to nurture positive ideals. Participants meet weekly for at least one hour. Mentors commit for at least one year and are required to undergo a screening process that includes FBI criminal history, driving record, child abuse and sexual offender registry checks. Mentors are also required to attend an initial 2-hour training prior to being matched. Call for further information.

Provides a detoxification center for anyone who needs monitoring or supervision while withdrawing from the absence of alcohol or drugs from their system. Also provides counseling for those with an immediate crisis due to alcohol or drug abuse.

An adult day program for seniors who need some supervision and care, with services of social and recreational activities, and training that is enhances daily living activities.

This class is for parents/caregivers of children who are easily frustrated, extremely inflexible, lose control, and become verbally and/or physically aggressive. The course is based on the work of Ross W. Greene, Ph.D., author of The Explosive Child. Caregivers will learn how to understand and handle explosive behaviors and prevent meltdowns, establish realistic expectations, improve parent interactions, and how to help their child recognize, express, and think through frustrations.

Program that is provided for accessibility modifications and emergency home repairs for the primary residence of veterans with disabilities. Eligible veterans may be renters or homeowners, with low and moderate income, with service related injuries or health related disabilities.
