The HNP team is staffed by a Health Educator-Manager, and Health Technicians. Technicians assess homes during a visit, provides recommendations and free products to improve health and wellbeing. Products may include smoke and carbon monoxide detectors, asthma relief products, and cleaning supplies.
The HNP is not an enforcement agency. All identifying information gathered during visits will remain confidential.

Psychiatric day treatment program for youth in grades 6-12 that may benefit from enhanced clinical care within an academic setting. Provides educational and clinical services to students in a 6:1:1 classroom setting at a standalone facility. Students receive educational services along with individual therapy, group therapy, family therapy, and medication management when necessary. Students can also access crisis support services when therapeutically necessary.
Integrated mental health services are provided within a strength-based recovery model. Partners with Erie 1 BOCES component school districts, Buffalo Public Schools, and families in the referral process, and collaborates with both families and school districts to reintegrate youth into a less restrictive academic setting. Also assists with linkages to community support services to support recovery. Average length of outpatient treatment is 3-6 months or as soon as the student meets their established treatment goals to be discharged to a less restrictive setting.

Personal Care Services Aide Program: Provides some or total assistance with personal hygiene, dressing, and feeding; nutritional and environmental support functions and health related tasks. Such services should be essential to the maintenance of the patient's health and safety within his/her own home, ordered by the attending physician based on an assessment of the patient's needs by a qualified person in accordance with a plan of care, and supervised by a registered professional nurse.
Consumer Directed Personal Assistance Program: New York State recognizes that some patients who need personal assistance also have a strong desire to manage their own services. Consumer Directed Personal Assistance allows patients to screen, interview, hire, train and supervise their attendants.
