Provides permanent housing and case management services for formerly homeless, chemically dependent individuals and their families. The program provides support so that eventually the individuals and families can assume their own lease and become fully independent.

Program is available to those individuals who require more support, but no longer need a community residence. Staff members visit on an as needed basis and assist in medication monitoring and additional services. Must meet HUD criteria.

The hotline is intended to assist homeless Veterans and their families, VA Medical Centers, federal, state and local partners, community agencies, service providers and others in the community.

Provides homeless single men and women who are Town of Huntington residents with temporary shelter during winter months.

Offers a hotline to ensure that homeless Veterans or Veterans at-risk for homelessness have free, 24/7 access to trained counselors. The hotline is intended to assist homeless Veterans and their families, VA Medical Centers, federal, state and local partners, community agencies, service providers and others in the community.

Provides limited emergency food (no perishables) and referrals to emergency shelters (cost is shared with other agencies if necessary). Also provides financial assistance to the immediate parish families only when needed. Offers Bereavement Programs or referrals to programs (suicide, death of children, widow, widowers, miscarriage and stillbirths) one on one Employment Network and various other services. During November 1st through April 1st homeless guests are screened and transported to one of our 14 host Houses of Worship between Greenport and East Hampton. Upon arrival homeless guests are registered and served a hot dinner. At day break breakfast is served, in addition a bagged lunch is given to each individual as they are transported back to where they originated from.

Provides individuals with skills and strategies to develop self-empowerment, assists with goal setting and provides local mentors. Also offers outreach services to families of program participants.
