Offers veterans, including those with PTSD, support meetings, networking opportunities, creativity and job preparation workshops and social activity experience to promote a healthy transition back to civilian life. A particular focus is placed upon helping veterans showcase their talent and share their experiences through the arts, art shows, photography, music, dance and theatrical events.

Toll-free Information and Referral Line serves hundreds of family members and service providers each year. Staff has access to extensive resources across New York State to provide individuals with the information they need. Staff provides technical assistance to families, advocates, service providers and policymakers. Provides comprehensive mental health services to patients in their own homes. Offers specialized programs for patients of various ages. Arranges for other services for patients as needed and advocates for patients/families. Provides evaluation with feedback to medical doctor or treatment team, medication administration/education, behavior modification, general health/nutrition status assessment. There are currently nine separate apartment sites throughout Nassau County. Each apartment is centrally located allowing for easy access to public transportation, shopping, and laundry facilities. Each apartment is occupied with 2-3 adults of the same gender. They are fully furnished, along with appliances and heat/air conditioning. This treatment program is meant to foster and promote independence, along with daily support from the professionally trained staff. If needed, residents do have 24 hour access to staff.

The Child Single Point of Access (C-SPOA) is a process that assists children and youth, 0-21, who are struggling emotionally and/or have a mental health diagnosis and their families to access many in-home and community-based services and supports to prevent hospitalizations and out of home placements.

Members include clinicians and researchers who treat and study individuals with anxiety disorders and their families, and other interested individuals. Provides a therapeutic recreation program including group therapeutic activities for children with a serious emotional disturbance. Fosters better socialization skills and offers arts and crafts, sports activities, and field trips.

Provides a court ordered service for the adult clients who are non-compliant with psychiatric medication and as a result decompensate and are deemed unable to live safely in the community. The initial AOT order lasts for six months and if renewed the order could last for another year. Mandated services include, but are not limited to, medication management and case management or ACT. The Nassau County Department of Mental Health is required to maintain oversight responsibility for all clients on AOT orders. Call for additional information and inquiry.

Offers an array of home and community-based therapeutic skill building and family support services for children under age 21 with mental health and/or substance abuse needs. Recipients must have Medicaid.

Offers outpatient psychotherapy services to women and men who suffer with anorexia,bulimia, compulsive overeating, binging, and body image problems. Provides continuing day treatment mental health services to Hispanic/Spanish speaking clients. Services include: assessment, treatment planning, individual and group therapy, vocational/educational training, recreation, crisis intervention, rehabilitation services, social support, self help and empowerment services for individuals 18 years or older.

Provides both vocational and community integration services and serves as a job placement center for special needs populations in Nassau County. Our program provides outreach and/or mentorship to those with severe persistent mental illness and co- occurring disorders that may also include co-morbid health issues. The overall mission is to assist those in our program to enter the workplace as quickly as possible. We work with our program members to achieve increased community integration and independent functioning to prepare them for work. During the course of the program they may experience different levels of integration and independent functioning: developing work readiness, volunteer placement or actual competitive employment. This service replaces the Targeted Case Management Services. This agency contracts with Adirondack Health Institute (AHI) and provides linkage and supports to assist individuals with their mental and physical health care needs. OMH funded and contract with Health Home and DOH.

Offers a community residence specializing in substance abuse and mental illness treatment for residents of Suffolk and Nassau Counties. This program focuses on stabilizing acute symptoms of mental illness and helping client develop capabilities and support necessary to lead fulfilling, drug free lives by addressing daily living skills, socializations, symptom management and community reintegration. All referrals for this program must go through Single Point of Access. Licensed by the New York State Office of Alcoholism and Substance Abuse Services (NYSOASAS).

Funded by the New York State Office of Mental Health, this program is intended to provide Non-Medicaid Care Management and Care Coordination services to adults diagnosed with a serious mental illness (SMI). The primary goal of these services is to help individuals maintain community tenure while improving the overall quality of their recipients' lives. Services include: referral and linkage to medical and mental health services and providers to obtain optimal health and wellness, linkage to housing and other social services, obtaining entitlement benefits and application for Medicaid, on-going assessment and the development of client specific care plan, monitoring established goals, case specific advocacy, and a liaison to the Suffolk County SPOA Unit. The Sayville Project is sponsored by the School of Social Welfare at Stony Brook University.

The Case Management program is designed to serve severely and persistently mentally ill and seriously emotionally disturbed adults. This 24-hour-a-day program provides aggressive outreach and support to individuals who either overuse emergency room and inpatient services or drop out of services. Access is by referral.
