Provides core services including Intensive Care Coordination-assessment, goal planning, linkage to community resources and coordination of service plan to help youth and family function in their natural environments; and Crisis Response for 24-hour call intervention. Referrals are made through SPOA (Single Point of Access). Services are provided in the home and community of the client.
