Referrals are made through SPOA (Single Point of Access). Services are provided in the home and community of the client.

Empowers community reintegration for individuals on parole, probation, or formerly incarcerated by providing comprehensive support services aimed at substance use and mental health recovery. Services offered include: Substance Use & Mental Health Screenings, Counseling Services and Recovery Support, Case Management, Peer Recovery Support, and Goal Planning. Free resources and ongoing programs are tailored to aid in successful community reintegration and family reunification post release.

Offers veterans, including those with PTSD, support meetings, networking opportunities, creativity and job preparation workshops and social activity experience to promote a healthy transition back to civilian life. A particular focus is placed upon helping veterans showcase their talent and share their experiences through the arts, art shows, photography, music, dance and theatrical events.

Helps resolve and prevent racial and ethnic violence and disorder, and to diffuse potentially explosive situations in schools and communities through mediation and conflict resolution. This regional office services New Jersey, New York, Puerto Rico and Virginia.

Offers both group and individual counseling as well as educational resources for caregivers of an aging or elderly loved via Zoom and in person. Weekly counseling sessions are available to help the caregiver handle their stress, better understand their role and options, and secure any assistance that maybe available. Educational sessions are provided to community groups and businesses in a “lunch and learn” format, with topics such as dementia care, elder law, nutrition and long-term care planning. Groups are specialized to meet the needs of adult children and spouses of those with loved ones who have dementia. SERVICES ARE FREE OF CHARGE. The program is funded by the Department of Health and Human Services through the New York State Office for the Aging and the Suffolk County Office for the Aging.

Team of professionals that help individuals e.g., paramedics, police officers, disaster workers who have been involved in emergency operations under conditions of extreme stress or people who have experienced or witnessed a traumatic event such as workplace or school violence, recover from the traumatizing effects of the experience utilizing a therapeutic technique which enables them to process the event and put it behind them. A CISD enables workers to discuss what happened, their role in the event, the impact of the experience and skills for coping with the after-effects. A CISD will generally alleviate the acute stress responses which sometimes appear at the scene or immediately thereafter and will at least inhibit delayed stress reactions which can appear days, weeks or even months later.

Provides individual and group bereavement counseling, child abuse counseling, crisis intervention to people experiencing stressful situations such as death of a family member, separation or divorce, severe accident or illness, surgery, rape, child abuse, loss of job or home.

Provides a free, confidential, 24 hours mental health service for anyone who is experiencing a mental health crisis. The mobile team can go anywhere in the community to evaluate and provide crisis intervention and referrals to services and agencies as required.

Provides a caring community that understands the challenges of living with diabetes. Helps individuals to obtain knowledge, skills, and support to better manage prediabetes and diabetes. Programs include:

• Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes.
• Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
• Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
• Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.

Offers respite for caregivers who need a temporary break form taking care of a family member with mild to moderate cognitive impairment, dementia, or Alzheimer's. Provides stimulation and socialization through art, music, recreational therapy, reminiscing, trivia, chair yoga and exercise. Call for further information. Appointment required.
