With activities and curricula available for all ages, students are empowered to end social isolation in three easy steps: See Someone Alone -- Step 1: Learn how to recognize the signs of loneliness and social isolation; Reach Out And Help -- Step 2: Find out what you can do to help others feel included and Start With Hello -- Step 3: Discover how to break the ice and strike up a conversation. Offers a skill building group therapy program for children and adolescents who struggle with social, emotional, and/or behavioral difficulties,along with a family support group.

Provides a unique program designed for adolescents and their family who are committed to losing weight but who have had poor results with conventional diets or summer camps. The goals of the program are to help pre-teens and teens reach and maintain their ideal weight while making healthy lifestyle changes. ON TARGET offers a medically supervised 12 month program that promotes weight loss, and enables the family to develop a supportive home environment, which significantly improves the chances of long term success.

Provides the following services for children and adults with developmental disabilities:

• Parenting Skills Training for parents of children with developmental disabilities, ages birth to 21 years
• Respite (Day/Evening) for children/youth under 21
• Saturday Life Skills Program for children with developmental disabilities and their families; 10 week program held 4 times a year
• Adult Day Habilitation Programs: specialized community-based programming for adults with developmental disabilities
• Vocational and Employment Services offers support for individuals with developmental and other disabilities and delays who are seeking employment including pre-vocational training and supported employment
• Behavioral Health Services provides short-term interventions for children and young adults struggling with behavioral, social, and emotional challenges
• Feeding Clinic for children from birth to age 12 with feeding and mealtime concerns

Provides early intervention services for children with developmental delays. Evaluates and provides speech, occupational, physical therapies, and provides social work services, psychological and special educational services, including planned behavior modification for children with autism. One-to-one services are also available for parents who can or want to pay.

Provides behavior management, counseling, crisis intervention, family reimbursement, guardianship assistance, benefits and entitlements assistance, home modification, independent living skills, information and referral, parenting skills for parents of developmentally disabled children (in or out of home), service coordination/case management, support groups, and transitional planning for persons with developmental disabilities of all ages. Developmentally disabled parent training is provided to first time parents with a developmental disability. Applied Behavioral Analysis (ABA) Program available for parents of children with autism.

Provides individualized assessment and intervention services to individuals diagnosed with autism and other developmental disabilities who display challenging behaviors. Provides school consultation and behavior therapy services.

Offers intensive clinical services which may include occupational and physical therapy, speech and behavioral services to individuals ages 21 and older with severe and profound levels of intellectual and other developmental disabilities. Offers intensive sensory integration programming and other planned activities within a hub based location as well as environmental exploration within the community.

Works to empower individuals with developmental disabilities, youth and adults with chronic health conditions and complex mental health concerns, and survivors of domestic violence to live as independently as possible. Adult Services include:

• Day Services, including site-based day habilitation, community-based day habilitation, and Individual Retirement Experience (IRE) for those aged 50 and over
• Employment Services which help develop work skills and connect people with competitive employment opportunities
• Self-Directed Services, which enable individuals to make choices about their staff, supports, and services

• In-Home Services, which provide home and community-based skills and self-development training services to adults who exhibit deficits in independent living care skills, with a focus on increasing or maintaining skills to reduce reliance on family caregivers

• Representative Payee Services, which help people to build their financial skills with the goal of financial independence

• Raise the Age Services, which helps youthful offenders acquire the life skills and pro-social behaviors required to lead self-sufficient lives, gain employment, and avoid further involvement in the criminal justice system
• Project C.O.A.C.H., which provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families
• Domestic Violence and Victim Assistance Program, which provides services to people of all abilities who are victims of any crime, specializing in people with disabilities who have experienced domestic violence

Provides a six session skills development program for parents. Participants will learn to reduce child problem behaviors, decrease yelling and fighting, learn how to stay calm and help their child develop self control, enrich relationships with their children, and increase their own self-confidence as well as that of their children. Day and evening classes are available in several locations. Call for more information.

Offers a 12-week Target Fitness Weight Management program for adults with weight management or other health related issues such as Type 2 Diabetes, Hypertension, or High Cholesterol. Program strives to guide participants to develop research-based skills to facilitate gradual and permanent weight loss through individualized strategies and goals. Registration required. Offers counseling and medication management to individuals 5 years of age and older. Offers group counseling group for anger management.

Provides Family Support Services as well as Waiver Services to eligible participants. Provides Family Reimbursement, Behavior Management (not intensive), and Service Coordination and Referral. Also offers Recreation/Respite and Community Habilitation Services.

Offers individual, family and group counseling, behavior management as well as information and referral for individuals with intellectual and developmental disabilities and autism and their families. A therapeutic, adult, social, and recreational club meet weekly to work on goals that include increasing socialization and self-advocacy skills. Also provide a transition program for adolescents going to adulthood, including case management for children and adults with developmental disabilities.

Provides people with the necessary services and supports to pursue their personal interests, learn valuable life skills, and attain their goals. Aims to assimilate these individuals into their local communities. Staff members set up and accompany day trips, volunteer projects at many Long Island-based charities, and work directly with select individuals to help them attain employment through People's Arc of Suffolk's Vocational and Supported Work Programs.

Offers a number of programs designed to meet the needs of children with developmental disabilities and their caregivers. Services include:

• Respite Services: Offers planned short-term and time-limited opportunities for families and care providers to have some personal time.
• Employment Services: Helps adults and youth to obtain and retain competitive employment through vocational counseling, employment training programs, work experience acquisition, soft skills development, rehabilitation services, and support services to help youth obtain, maintain, or advance in competitive employment or self-employment.
• Children's Health Homes/Care Management: Includes care coordination, comprehensive transitional care, individual and family support, and referral to community and social support services as needed.
• Community Habilitation: Assists youth living at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them.
• Community Based Day Habilitation: Promotes community inclusion and integration.
• Family Reimbursement Program: Covers expenses for goods and services needed to care for a loved one with a developmental disability who lives at home.
• Self-Directed Services: Enables individuals to make choices about their staff, supports, and services.
• Preventive Services: Provides support to help families learn the necessary skills to ensure their children remain at home and out of foster care placement.
• Behavioral Health Services: Includes community habilitation, caregiver/family supports and services, pre-vocational services, and supported employment.
• Project C.O.A.C.H.: Provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families.

(PBIS) services are provided to participants who have significant difficulties. The primary goal of PBIS services is to decrease the intensity or frequency of targeted behaviors and to teach more socially acceptable behaviors.

Provides residential options and one-to-one support to focus on increasing independence in practical skills such as cooking, shopping, banking, an environment focused on task-oriented activities and social skill building, direct technical assistance to individuals with challenging behaviors, counseling to participant or family members to assist with issues related to community integration, adaptations to residences to promote independence and to ensure safety.

Provides people with the necessary services and supports to pursue their personal interests, learn valuable life skills, and attain their goals. Aims to assimilate these individuals into their local communities. • Anxiety disorders, including Obsessive Compulsive Disorder (OCD), generalized anxiety, panic disorder, specific phobias, social anxiety, separation anxiety, and school phobia
• Disruptive behavior disorders including Attention Deficit Hyperactivity Disorder (ADHD), Oppositional Defiant Disorder (ODD), conduct disorder (CD), and general behavior problems
• Elimination disorders including enuresis, nocturnal enuresis, and encopresis
• Mood disorders including dysthymia and depression
• Pervasive developmental disorders including Autism, Asperger's Disorder, and PDD-NOS
• Others conditions including tics, Tourette's, habit disorders, thumb-sucking, sleep problems, selective mutism, tantrums, defiant behavior, and social delays
