Lymphedema screenings are held throughout the year. The Center also offers an early detection program for patients at the Fortunato Breast Health Center. Offers the only support group in Suffolk County. Located in Suite 301.

- Diabetes Prevention Program: For those with prediabetes or at high risk for type 2 diabetes. A year long program focused on implementing lifestyle changes to reduce the risk of developing type 2 diabetes.
- Diabetes Counseling: For individuals with prediabetes or diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support.
- Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications.
- Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.
