Offers a co-ed group home that provides long-term residential placement to adolescents ages 12 to 21 years old in the care of and custody of a county Department of Social Services. Programs provided include assessment, case management, individual and family counseling, outreach services, medical services, special education, coordination with public schools, recreational therapy, emergency placement, independent living skills training, transportation, discharge planning, and transitional support.

Community residence for adolescents males and females who are in need of structured setting in the community that enables them to become involved in the responsibilities of daily living due to a serious emotional disturbance(SED)

Provides complete care for a total of eight co-ed youth, ages 12-17, for an extended period of time. Program services focus on individualized service care plans, education, therapy, work, volunteer activities, daily and independent living skills instruction, medical management, and self-awareness of youth disabilities. The goal for each youth is to establish and achieve personal growth that will enable them to return to their family, independent living, or chosen housing goal as soon as appropriate.

Lee Randall Jones Community Residence, certified by the Office of Mental Health, is an eight bed residence located in Cheektowaga. The Community Residence provides services to children ages 5-14 with a primary psychiatric disability that does not require acute care but may still need the structure and support of this community based program. The program provides 24-hour supervision paired with community-based medical, recreational, and psychiatric services that may continue once the child is discharged home.

Provides services to needy, troubled, and emotionally disturbed youth populations under the age of 21. Community Residence level of care provided for children and youth who are referred through the Office of Mental Health. Juvenile Justice Outreach and case management programs provided for families and youth referred through NCDSS and Niagara County Probation Departments. ##REENTRY##
