They serve Medicaid-eligible individuals needing 24 hours a day - 7 days a week care from staff, who work intensely with the residents in formulating individualized service plans. These generally include medication management and training, life skills training, and services and interventions that support health and wellness.

This facility is licensed as an adult home providing long-term residential care with 24-hour supervision and a full range of services such as meals, housekeeping, some personal care and social activities. Adult homes are licensed and regulated by the New York State Health Department.

Residents of the Apartment Treatment Program generally require less staff support than in the Supported Housing Program. Staff visits vary depending on the needs of residents. Usually, counselors visit residents 3- 5 times per week. However, visits can be increased should residents need additional support. All residents have 24-hour telephone access to staff in the event of an emergency. The Apartment Treatment Program provides residents with the opportunity to live independently, yet still have the benefit of frequent staff support. Residents work on areas needed for independent living such as, symptom and medication management, accessing community resources, and budgeting. The Apartment Treatment locations are situated throughout Suffolk County, Long Island, NY. All provide easy access to public transportation, stores, recreational facilities, and services. All apartments are private homes, which provide individual bedrooms to residents.

The main goal of this program is to increase independence while providing support to the person and the family. People are encouraged to strive to live independently in the community with minimal staff support.

Aids individuals who have a Section 8 certificate or who have progressed through SFC's system of care, by assisting in the process of choosing an apartment, facilitating the lease, and maintaining their independence within the community. Weekly in-home and phone support are provided based on choice and individual needs. Monthly support groups and recreational/social activities also provided.

The Apartment Program offers a less intensive level of care for persons recovering from mental illness as participants reside in either 1 or 2 bedroom apartments located within the City of Plattsburgh. All residents have their own bedrooms, however, those living in 2 bedroom units share common space such as kitchen, living room and bathroom. Case Managers meet with residents throughout the week to offer rehabilitative support services.

Provides a variety of residential programs for individuals with mental illness which are designed to improve or maintain an individual's ability to live as independently as possible and eventually access independent housing. Interventions are provided consistent with the resident's desire, tolerance and capacity to participate in services.

SHP provides subsidized housing and case management for adults with mental illness or developmental disabilities.
