Transition specialists will meet with you and/or your family members to identify what services you will need and help you in getting those services.Open Door's Peer Program is a group of people who have lived in a nursing home and intermediate care facility and are now receiving services and support locally available to them. Peers visit nursing homes and developmental centers to share their experiences with residents and support people who want to move to the community.

EISEP is a non-medical service program for elderly citizens. Services are provided to make the home environment safer for the older person in need, and may include a personal emergency response system.

If you are ineligible for DSS services, please call Long Island Coalition for the Homeless at (631)464-4314. Provides food, clothing, and shelter for the homeless on the East End of Long Island. Overnight emergency shelter is open from November 1st to April 30th. Guests are transported nightly from Maureen's Haven to an assigned host site at locations in Southampton, Southold, Riverhead, and East Hampton. The Kay Kidde Day Center, Case Manager, and Support Services are available year round at the Riverhead location. Services include: case management, housing assistance, access to healthcare providers, counseling, supportive services, programs, daytime respite, food, clothing, job training, educational opportunities, and a community garden. The Homeless Outreach Programs provide critical services to those who need it most. They include: the Summer Weekend Lunch Program, street outreach and responding to concerns about the homeless in our community, and coordinating services among providers for those individuals who are unable or reluctant to access them directly.
