Screenings are provided by a Social Care Navigator, a trained professional who guides Medicaid members through the process of addressing their identified social needs. Navigators may provide Eligibility Assessments, Social Care Plans, referrals, care coordination, and follow-up. Over the phone translation available for more than 180 languages.
