Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

Provided onsite to residents: activities, dental services, dietary services, housekeeping services, health services, nursing services, occupational therapy services, physical therapy, physician services, podiatry, social work services, speech/language pathology services and diagnostic x-ray services. Provided offsite to residents: clinical laboratory services, dental services, mental health services, pharmacy services, podiatry services, and diagnostic x-ray services.

Provides comprehensive outpatient health and rehabilitation care, as well as a supportive group environment with recreational activities. Health services provided include nursing care, personal care services, audiology, speech therapy, health monitoring, nutritional services, occupational therapy, physical therapy, case management, and health care monitoring of blood pressure, blood sugar levels, medication, and other health regimes. The following services may be arranged as needed: laboratory services, radiology, dentistry, ophthalmology, podiatry, and beauty/barber shop visits. Offers services to individuals with traumatic brain Injury, multiple sclerosis, strokes, Parkinson's disease, diabetes, hypertension, intellectual disabilities, and behavioral health issues.

Offers day program providing same services as a nursing home. Serves frail/ill/handicapped persons living at home. Offers physical/occupational/speech therapies,dietary/podiatry/dental/optometry services, and medication management. Conducts therapeutic recreational activities to provide stimulation and encourage socialization. Serves two meals, assists with activities of daily living, offers wellness programs, hair care services. Provides transportation. Offers family support/counseling.

Offers two (day and evening) five hour a day programs for individualized medical and nursing care. Provides social work services, therapeutic recreation, rehabilitation, nutrition counseling, dental services, podiatry services, medication counseling, audiology screening, socialization, hot lunch and transportation. Serves frail elderly and disabled who are unable to function independently in the community, physically or mentally impaired, socially isolated or those who need assistance with personal care.

Medical model adult daycare program for older adults in need of daily supervision and health services.

Provides a 7-day a week adult day program which includes nursing care, physical, occupational and speech therapy, social work, recreational activities, outings, meals, nutrition education, and wheelchair accessible transportation.

Offers volunteer opportunities for ages 14 and up including recreational activities, pastoral care, clerical assistants, day care assistants and immigrant services.
