Operates Article 16 Diagnostic/Treatment Center providing assessment, evaluations and treatment, and family support services through staff of physical and occupational therapists, speech pathologists, nurses, social workers, psychologists, and medical consultants.

The purpose of this program is to identify people in nursing homes who wish to transition back into the community, explain programs and services available to them, and make the appropriate referrals to begin the process.After their transition, participants are offered Person Centered Coaching for 6-8 skill building sessions to help identify strengths, preferences, needs, and desired outcomes. Open Doors also works closely with the Olmstead Housing Subsidy (OHS) Program, which provides housing assistance, furniture, and household goods to those who qualify.

Assists families by providing one-on-one education and support, assessing patient and caregiver needs and helping secure services at home.

Assists individuals with disabilities to find services and support they need to live in the community rather than in a nursing facility or to prevent institutionalization. Provides referral and information, networking, technical assistance, provider development and service linkage to eligible individuals and their families.

Assists individuals who are willing and able to get out of a nursing home and live in the community. Services include case management to make sure each individual has access to the care they will need.

Provides an array of mental health services, such as mental evaluation and assessment and coordinates the mental health needs of the diagnosed individual, along with the daily living skills. Also available is smoking cessation for those who want to stop smoking. Social workers help patients and their families deal with various issues dealing with age, chronic illness or disability.

Assessments and advise about options to seniors and families about future arrangements for care. Also offers a service to seniors who are not yet frail to create a future plan to avoid any care giving crisis. . Offers volunteers who help with home visits and advocacy, compassionate phone calls, relief for caregivers, grocery shopping driving/assistance, yard/home maintenance (late teen and older only, ask for Dennis), drivers for medical appointments and fingerprinting services.

Offers community intergenerational activities; pre-vocational skills training; employment preparation; activities of daily living training.

Provides assessment and training which is individually designed based on the needs and abilities of adults with developmental or acquired disabilities to live as independently as possible in the community. Training and development may include medication management, task completion, communication and interpersonal skills, socialization, sensory/motor skills, mobility, community transportation, problem-solving skills, and money management.

Helps with housekeeping or personal care so older adults can stay in their own homes for a longer length of time.

Offers outpatient support services for any individual who has gone through a colostomy, iliostomy or other ostomy care, wound ostomy and continence. A nurse evaluates each patient's needs and helps them deal with everyday effects, learn to live with it, and to lead a productive lifestyle.

HCBS services are for children and youth who need extra care at home/in the community, want to avoid going to the hospital or a long term facility, need support as they work toward goals and achievements, need personal, flexible services to meet the health, mental health, substance use treatment and/or developmental needs.

Provides comprehensive assessments of an individual's home health care needs, coordinates the most appropriate effective services, and provides case management. Designed to help health impaired Erie County residents remain in their homes. Provides linkage for clients with home care needs including personal care, meals, and housekeeping. Screens for Medicaid eligibility for home care.

Assists elderly to remain in their homes and to participate in family and community life as long as possible.
