Offers case management services, bilingual and minority services, harm reduction education, referrals to housing assistance, client advocacy, emergency financial assistance, home delivered meals, a PLWH/A support group, prevention education and client recruitment outreach for those infected and affected by HIV/AIDS. Offers diversity outreach, alcohol and substance abuse education programs, home delivered meals to those infected and affected by HIV/AIDS, HIV CTR and STI CTR. Call for further information.

Provides HIV and HEPC education, screening, rapid testing and linkage to care. STI education and testing also available (not Rapid). In addition, care coordination services for chronic diseases such as diabetes, hypertension, obesity and depression, case management and nutrition services for HIV are offered.
