MEDICAID SERVICE COORDINATION | LIFE'S WORC

Provides case management and coordinated support services, which include advocacy, referral and resources through the Home and Community Based Medicaid Waiver program, to developmentally disabled individuals 18 years or older who are medicaid eligible and meet criteria for participation in OPWDD programs.
