Serving veterans who were previously homeless, who are transitioning into permanent housing from VA or non-VA emergency or transitional housing programs. Available services include assessment, plan development, identifying available resources, advocacy, contact and communication, and coordination of services. Referrals can be made if a veteran has not yet moved into identified permanent housing, or if they have been living in permanent housing and are at risk of losing the housing. Veterans eligible to enroll in this program must qualify for VA services and cannot be receiving case management services from any other program, i.e., HUDVASH, SSVF, Care Coordination.
