Required documents
Volunteers must be age 16 and over, PPD required and photo ID. Parental consent is required in writing if LDDSO Clinic is giving PPD (Tuberculosis Test). Volunteers must be residents of Nassau County or the following towns in Suffolk County: Town of Babylon, Brookhaven, Huntington, Islip, Riverhead, or Smithtown.
