Eligibility

Low income pregnant, postpartum, and breastfeeding women and their children up to age 5. Must reside in New York State. Applicants must meet income eligibility guidelines or receive Medicaid, Snap(Food Stamps), or TANF

Required documents

Proof of identity of child,proof of household income, proof of residency, immunization record for infants & children. A completed New York State Department of Health Medical Referral Form from your health care provider(with blood work or proof of pregnancy)
