Intensive Case Management and HCBS Waiver for Children with Emotional and Behavioral Issues | GREATER MENTAL HEALTH OF NEW YORK

Provides individualized, community-based care coordination and case management for children with serious emotional disturbance, which threatens a family's ability to keep the child at home. MHA utilizes a strength-base model that promotes wellness, leading to success for the child and family, along with care coordinators who collaborate with the family, treatment providers, and other community-based support to identify a support team for the the family and advocates for needed and appropriate services that promote the successful care of the child in the home and community.
