Provides assistance to those in recovery, facing homelessness in Suffolk County. Peer outreach counselors assist each person to help them re-establish hope, take charge of their situation and get back on track. Services are mobile and can be provided in any setting where a person is most comfortable. Individuals receive peer support and are assisted with linkage to resources such as housing, self-help, mental health services, substance abuse, medical services, financial assistance, legal services, and employment and benefits as needed.
