Provides community based treatment, rehabilitation and support services for adults with serious and persistent mental illness, such as schizophrenia, schizoaffective disorder or bipolar disorder, through a multidisciplinary approach to help manage symptoms, crisis response, case management, peer support, medications, supportive therapy and practical support in coping with day-to-day demands.
