A cost share may apply to EISEP services which is determined by a case manager during the assessment process. No one, however, is refused these services because of an inability to pay.

This program connects Nassau County seniors with services through government agencies, parishes, and community-based organizations. A case manager works with each senior to assess needs and develop and implement a service plan, which might include home care, state and local entitlements, and other needed services. This program provides: Case management, Connections to community support services.
