MEDICAL ALERT PROGRAM | TOWN OF SMITHTOWN

If you are a Smithtown resident age 60 years or older, you are eligible for our Medical Alert Program.

Proof of age and town residency.

Joining this program requires a brief in-home assessment. Following this assessment, a technician from Connect America will visit your home to deliver/install equipment and provide instruction on usage.

The home-based service is available for a monthly monitoring fee of $18. The GPS enabled system is available for a monthly monitoring fee of $30. Monthly monitoring fees are paid directly to Connect America.
