Eligibility
Adults ages 18 and older who meet eligibility requirements for Chautauqua County Department of Mental Health Single Point of Entry (SPOE). Must have had prior inpatient treatment, including a minimum of two hospitalizations during the past two years.

Office: Monday-Friday, 8am-4pm.
Emergency on-call services available 24 hours a day, 7 days a week.
