Eligibility

Children and youth ages birth to 21 who are enrolled in Medicaid and have one or more of the following: at least two chronic conditions, Diagnosed Severe Mental Illness (SMI) including Severe Emotional Disturbance (SED), or HIV. Pending approval from CMS, "trauma, and at risk for another chronic condition" will be added to these eligibility criteria. Service Area: Allegany & Cattaraugus Counties.

Children's Health Homes was established to plan for and operate as a Health Home for Children. A Health Home is a care management model in which all of an individual's caregivers work together and share the information necessary to support recovery and health. This model works on family strengths and ensures that families have "voice and choice" at every stage in the care management process. CHHUNY was created in order to provide high quality, family driven care management to youth with health needs by partners experienced in providing care to children and their families. This is done primarily through a care manager who oversees and provides access to all of the services the child needs to ensure good health.
