LONG ISLAND CENTER FOR DIGNITY AND SUPPORT (LICDS) | JEWISH ASSOCIATION SERVING THE AGING (JASA)

Provides in-home, web-based and telephone counseling services. Seeks to keep older adults living independently by addressing challenges as well as supporting those with serious & advanced illnesses of all ages through person-centered trauma informed services. LICDS services include care management (navigating, healthcare system, advocating for timely & appropriate care, accompanying clients to medical appointments, information & referrals, entitlements & emergency assistance), counseling (coping with a serious illness & end-of-life issues, issues of aging, caregiver support, goals of care, bereavement, spiritual care, & advance care planning), nursing services (in-home assessment, medication management, counseling, care coordination, fall prevention & education). LICDS also offers community & professional education on end of life, advanced care planning, bereavement, care-giving and entitlements.
