Assists consumers with NYS Managed Long-Term Care (MLTC), Health and Recovery Plan (HARP), Long-Term Support Services (LTSS) in Mainstream Managed Care (MMC) programs, Transitioning into MLTC plan, FIDA-IDD Plans. Provides education to stakeholder groups, provides information and advice to participants, caregivers, and their advocates, provide navigational assistance, and provide advocacy when needed.
