Required documents

An application requesting ICM services must be submitted to the CAMERA Unit in the Suffolk County Division of Community Mental Hygiene. Must be a Suffolk County Resident, 18 years or older, diagnosed with ASPMI and demonstrate a functional impairment due to the mental illness.
