Open Doors Program | NORTH COUNTRY CENTER FOR INDEPENDENCE

The Open Doors Program assists individuals with disabilities who are living in or at risk of entering nursing homes or other institutional settings. The program helps people transition to, or remain in, the community by providing information, advocacy, and support to access housing, services, and long term support that promote independent living and community integration.
