Eligibility

Those with a diagnosis of incurable cancer with documentation to support: the diagnosis; the need for palliative care after all aggressive cancer treatment(s) [i.e. radiation, chemotherapy and surgery] have been completed; and financial need. Insurance is not accepted, but those with insurance are not automatically disqualified; individual circumstances are taken into consideration. There is no discrimination on the basis of race, creed, color, national origin, sex, or disability.

Required documents

Diagnosis of incurable cancer, a pathology report, CT Scan or similar documentation is required documents showing inability to pay for care a signed Do Not Resuscitate form (DNR), PPD Skin Test Results, a list of current medications, a signed Health Care Proxy, a completed NY DOH 695 screening form (if New York resident), and a medical history and physical exam or consultation (i.e. narrative report) may all be required.

The Home is free to all who meet the eligibility requirements. No payment is accepted from patients, their families, private insurance, or from the government.
