Eligibility

Families living in Erie County with a child between the ages of 5-17 years old who has a mental health diagnosis or severe emotionally disturbed classification and who is at risk of out of home placement.

Works to improve the lives of families raising at-risk youth faced with social, emotional, mental and/or behavioral challenges. Services are provided in partnership with the youth, their parents and/or caregivers, referral sources, and other sources of community support to prevent out-of-home placement of children. Program services include individual, family, and group therapies; skill building (for parents and youth); in-home community behavioral services; outpatient diagnostic assessments; and Rise and Shine, an after school educational enrichment program. All services are family centered and provide ongoing support towards family preservation.
