Eligibility

Ages 5-30 who are diagnosed with a serious emotional disturbance (SED) diagnosis, complex trauma, or have two chronic conditions. Child must have active Medicaid. Program serves Nassau County and portions of Queens and Suffolk. Call the program for referral information.
