Eligibility
Residents in licensed housing must be residents of Nassau or Suffolk Counties with a primary mental health diagnosis and a secondary diagnosis of substance use disorder if applicable. The independent housing programs serves both Nassau and Suffolk County residents with mental illness, as well as individuals and families with disabilities who are homeless. Provides an array of housing alternatives and staff supports to people with a diagnosed mental illness. The rehabilitativehousing program allows residents with a diagnosis of both mental health and substance use disorders gain the skills needed to manage their psychiatric symptoms, maintain their sobriety and perform daily activities needed to live independently. Services include assertiveness/self-advocacy training, community integration, daily living skills, health services, medication management, parent training rehabilitative counseling, skills development, socialization, substance abuse services and symptom management. The independent housing programs offer financial assistance, case management and staff supports to allow people to maintain permanent housing in the community.
