Eligibility
For individuals who have active Medicaid and are living with 2+ chronic health conditions (e.g., Substance Use Disorder, Asthma, Diabetes), one serious qualifying chronic condition (HIV/AIDS, Serious Mental Illness (SMI) or Sickle Cell Disease).

Helps support individuals in managing their chronic health conditions and coordinating hospitalizations and assist with increasing independence of our community members. Eligible individuals are assigned a Care Manager who will help the member to develop personalized care plan goals, support in coordination of their care, link to community resources, assist with securing, maintaining benefits/entitlements, explore housing resources, and facilitate communication between medical and behavioral health providers.
