Eligibility
For ages 21+ (Adult), and 21 and younger (Children) who are enrolled in a Medicaid Managed Care (MMC) Health and Recovery Plan (HARP) and found eligible after completing the NYS Eligibility Assessment. Also, people enrolled in a Special Needs Plan (SNP) may also be eligible for BH HCBS. Assists individuals through the process of securing and maintaining a job through the Intensive Supportive Employment/Prevocational Services. Also offers Education Support to individuals interested i advancing their education or attending a formal training program. Habilitation Services are also offered, and are geared to help individuals learn or regain skills needed to help them reach personal recovery.
