Home Health/Care Coordination Services for Adults | Rensselaer County Department of Mental Health

The Case Management program is designed to serve severely and persistently mentally ill and seriously emotionally disturbed adults. This 24-hour-a-day program provides aggressive outreach and support to individuals who either overuse emergency room and inpatient services or drop out of services. Access is by referral.
