Offers a program which is a free resource for seniors to get information or guidance when faced with health and care issues. The program provides assistance to older adults, families and caregivers who need help answering healthcare questions, identifying their needs, and accessing necessary services. Some of these services are: Social Interaction/Programming; Cognitive Programming; Nutrition; Benefits Assistance; Home Services and Housing/Placement. Stony Brook Eastern Long Island Hospital is working closely with Southold Town Senior Services and other local programs that serve seniors; and is committed to strengthening our community by improving the quality of life for older adults.
