The Long Term Home Health Care Program (LTHHCP) is a New York State Department of Health program administered by the local Department of Social Services. It is case managed by an R.N. who coordinates a plan (including medial, nursing, and rehabilitative and support services) to individuals in their homes who are medically eligible for nursing home placement. Also known as "Nursing Home Without Walls".
