Original birth certificate, current immunization record, health insurance and/or Medicaid card (if child has coverage) proof of income (W-2 from last year, Public Assistance Budget Sheet, 4 consecutive weeks worth of pay stubs, unemployment documentation, child support payments, SSI grant letter, foster care grant, tax form 1040), court papers verifying guardianship if applicable
