Program assists seniors 60 years and over to remain in their own homes after retirement. Services offered: healthcare monitoring and screenings, connections to benefits and services, home maintenance assistance and minor repairs, counseling, case management and service coordination, and nutrition assistance, as well as social and recreational opportunities.
